Registration Overview

The first step in registration is to complete a registration form. You may download a copy (2010) or call our office (816-697-2933) to have one mailed to you. The form, with your deposit of $100, should be mailed to the address on the form.

You will be asked to complete an Inforamtion and Consent form before you begin the training. A copy of this form will be mailed to you when you registration is received. You may also download a copy.

Complete the form and return it by mail, e-mail, or fax.
